The Healthy Family Teams offer basic assessment and low-level early interventions (Tier One) on emotional health and wellbeing issues and some other health issues.

The Healthy Family Teams referral pathway covers children and young people (CYP) attending Nottinghamshire schools or who live within the Nottinghamshire area.

Referrals can be made into the service using the Healthy Family Teams referral form, or via self-referral by a parent or carer by contacting our Advice Line.

Support can be accessed in various ways. This includes telephone contact via the Advice Line or one-to-one and group sessions.

A text messaging service called ChatHealth is also available to secondary aged pupils. They can confidentially get in touch with a health professional from the team. Support can also be accessed via Parentline. This is a text messaging service offering support and advice to parents.

 

How to refer someone to this service

Each Healthy Family Team will be able to offer support and advice to parents and carers of a child who:

  • is aged less than 19 years
  • lives in Nottinghamshire
  • attends a Nottinghamshire school
