AB Independent Investigation - September 2010

In response to the recommendations of the independent investigation, the Trust is taking a number of steps to improve the services that we provide to our patients. Please see below a short summary of what we are doing in response to each of the seven recommendations. A more detailed action plan can also be found at the bottom of this page.

The investigation’s first recommendation was that we should make sure that before one of our patients leaves our care (i.e. becomes an outpatient not an inpatient) the members of our staff that have been leading on their care should all come together to discuss and develop a plan for their care as an outpatient. We are currently on track to complete, in December 2010, an audit of our procedures to make sure that this is happening.
The investigation’s second recommendation was that we should review the way that we provide certain types of drugs to some of our patients. We have already fully addressed this recommendation.

The investigation’s third recommendation was that we should reassure ourselves that our policies and procedures for safeguarding children are being implemented properly. We are currently on track to complete this review in November 2010.

The investigation’s fourth recommendation was that we should make sure that whenever we set up an internal investigation to look at any serious incident that may have occurred, we ensure that the members of internal investigation are independent from the service where the incident occurred. We have already updated our policies and procedures and these will be formally ratified and implemented in December 2010. Monitoring of implementation will be reviewed in April 2011.

The investigation’s fifth recommendation was to ensure that where a serious incident may have occurred, the victims and families of those involved are supported. The investigation’s sixth recommendation was that we publish information on the way that our patients with a diagnosis of personality disorder are cared for (known as a care pathway). We have already made this information available on our website.

The investigation’s seventh recommendation was that we should evaluate how effective and accessible the advice services are for patients with a diagnosis of personality disorder.
