Community Stroke Team

Who Are We?
The Community Stroke Team provides rehabilitation to everyone who has had stroke, and has a Bassetlaw GP. We can see all patients for up to a year after their stroke, excluding any time spent in hospital.

Who is in the Team?
The team consists of a Physiotherapist, Occupational Therapists, Speech and Language Therapist, Dietitian, Psychologist, Social Worker, and Rehabilitation Assistants. Assessment and treatment by all therapists is available to each patient, if and when required.

How can we help?
Our aim is to promote independence, help people achieve their goals, and support/promote coping with emotional adjustment.

Who can make a referral?
If patients have been in hospital after their stroke, then they should be referred into the team by therapists at the hospital. Otherwise people can be referred into the team by health or social care professionals such as:
• GP’s
• Consultants
• Physiotherapists
• Occupational Therapists
• Social Workers.
The team works 8:30 – 16:30 Monday to Friday.
The patient must be diagnosed with a stroke within the last 12 months of their discharge from hospital, and they must be registered with a Bassetlaw GP.

What is the assessment process?
We carry out an assessment in the home to identify any needs patients may have as a result of the stroke. We then set individually tailored goals, and a plan of rehabilitation will be agreed and started. Patients are usually seen for periods of up 12 week, depending on their needs. A review is offered to all patients six months after their discharge from hospital, or six months after their stroke if not admitted.

We understand that a stroke can also affect a person’s family, friends, and/or carers. The Team will therefore also consider any further needs that families and carers might have, to help them cope with the impact of a stroke.
