Community Therapy Team

Who are we?
The Community Therapy Team provides rehabilitation for adults who have a long term condition or a short term condition following surgery and who are registered with a Bassetlaw GP.

Who is in the team?
The team consists of Occupational Therapists and Rehabilitation Assistants.

How can we help?
Our aim is to promote independence, to help people achieve their goals, and to support/promote coping with emotional adjustment. We do this by carrying out an holistic assessment in the home to identify any needs patients may have. We then set individually tailored goals and a plan for rehabilitation.
We understand that a long term condition can also affect a person’s family, friends, and/or carers. The team will therefore also consider any further needs that families and carers might have, to help them cope.

Who can make a referral?
The patient must be aged over 18 years and diagnosed with a long term or short term medical condition and they must be registered with a Bassetlaw GP.
If patients have been in hospital then they should be referred into the team by therapists at the hospital. Otherwise people can be referred into the team by health or social care professionals such as:
• GPs
• Consultants
• Physiotherapists
• Occupational Therapists
• Social Workers
• Self referral

The team works 8:30 – 16:30 Monday to Thursday

The team also manage the wheelchair service. This service and the criteria for accessing it is detailed separately.
