Who are we?
Bassetlaw Community Health Continence Service provides assessment of patients continence needs, advice/education an arrange home delivery to all individuals residing in the community, who are registered with a Bassetlaw GP who require simple or complex continence management, this includes individuals living in Nursing and Care homes.
Working closely with other health professionals, the specialist continence nurses undertake home visits to patients and providing on going advice and support to individuals to support their continence needs. If required, individuals may be referred to other services if clinically appropriate.

The services works closely with all members of children’s and adult community health teams, general practice and Hospitals within the area.

How to access the service
Referrals can by made via the Single Point of Access (SPA) on the following telephone numbers. Messages will be forwarded to the Continence Service

Who can Access the service?
Usually individuals are referred into the continence service from the list of professionals above, however self referrals are also welcome.
