Day Rehabilitation

Who are we?
The Day Rehabilitation Service provides time limited therapy input with identified outcomes. The aim is to maximise function, mobility and independence.

Who is in the Team?
The team consists of Physiotherapists, Occupational Therapists, Social Worker, and Rehabilitation Assistants. Assessment and treatment by all therapists is available to each patient, if and when required. The team works 8:30 – 16:30 Monday to Friday.

How can we help?
• Initial assessment to ascertain suitability to the service, including physiotherapy assessment and identifying needs and outcomes
• Group sessions to improve confidence and well-being, reduce risk of falls and maximise independence and mobility
• Community visits carried out for those that identify needs within the home environment
• 1:1 sessions for those that have specific ongoing therapy needs
• Transport provided if required.


Who can make a referral?
We accept referrals from patients, family members, other health and social care professionals and any other people involved in the patient’s care.

What group sessions do Day Rehabilitation provide?
Day Rehabilitation Service groups include:

Age Well
This is an 8-week programme for frail older people with reduced mobility/function. Focusing on exercise and education to reduce the risk of falls, improve confidence and well-being and independence. The programme aims to maximise mobility and improve co-ordination, functional activities and exercise tolerance. Patients should be able to sit to stand with assistance of one person and be able to actively participate in a group.

Staying Steady
This is an 8-week programme for patients who have fallen, or who are at risk of falling due to reduced mobility, strength and balance problems or general lack of confidence. The sessions focus on exercise and education. Patients should be mobile indoors and be independent in sit to stand. The group is a two hour session with a short break.

Both group sessions have a multi-disciplinary approach with identified patient goals. Following Day Rehabilitation Service input, patients are sign-posted to appropriate services where required.
