Who are we?
We are highly experienced team of people with Palliative Care expertise providing a service for people with a life threatening illnesses, including symptom control, psychological support, bereavement support, and pain management, the service is available to both patients with cancer and non cancer diagnosis.
The hospice has a 6 bedded in patient unit alongside a nine place day care unit and carers drop in sessions.
Who do we work with?
We work closely with the Hospice Charity and its Trustees, Palliative Care consultants, Oncologists, Medical Doctors, Community Matrons, District Nurses, Complementary therapists, Health and Social Care Services, Carers groups, Volunteers.
Who is in the team?
Nurses with Palliative Care experience and expertise. Macmillan Nurses. Doctors, Health Care Support Workers, Complementary Therapists, Catering Staff and Volunteers.
Day Care
Emotional, psychological support, information and advice following a recent cancer diagnosis
Patients undergoing either palliative or curative treatments (Chemotherapy, radiotherapy or surgical intervention)
Patients requiring emotional, psychological and social support at any stage of their cancer journey
Non cancer pathology patients with a life threatening illness eg end stage heart disease, COPD and neurological illnesses with clearly defined palliative needs.
Complementary therapies
An introduction to the hospice for people who are approaching end of life who may shortly require support from the In Patient Unit
IPU
Manage severe symptom control problems and pain management.
Psychological support.
Last days of life when home or hospital care is no longer appropriate or possible.
Last days of life when patient has requested hospice as preferred place of death.
Carer support group
Support and provide information to carers
Empower patients and their carers
Cancer survivor group
Bereavement support group
Who can make a referral?
Patients, family members and carers.
Health care professionals or Health and Social Care workers.
Referrals can be made through Single Point of Access on 01777 274422 or directly to the Hospice on 01777 863270.
Following a referral received from a family member or a friend we would then contact the GP or other health care professional for more clinical information.
