Parkinson's Nurse Specialist

Who are we?
The Parkinson’s Disease Nurse Specialist (PDNS) aims to provide people with Parkinson’s Disease access to clinical monitoring and medication adjustment; a continuing point of contact for support, including home visits when appropriate; and a reliable source of information about clinical and social matters of concern to people with Parkinson’s disease and their carers.

Who do we work with?
The PDNS work’s closely with primary care teams, community nursing, rehabilitation teams and intermediate care teams, GP’s, Neurological Consultants, Adult Social Care and Health and other specialist and voluntary services to ensure the highest possible quality of individualised care is provided.

How can we help?
• By providing support and advice for people with Parkinson’s Disease, their carers and professionals.
• By assessing, planning and evaluating programmes of care within the provision of specialist clinical nursing.
• By reviewing the prescribed medication regimen to ensure that patients receive appropriate pharmacotherapy and monitoring patients following medication changes.
• Liaising with primary care teams, community nursing, rehabilitation teams and intermediate care teams to facilitate the assessment, planning and evaluation of care for patients with Parkinson’s in order to avoid inappropriate hospital admissions and to develop flexible and innovative approaches to Parkinson’s specialist care.
• Liaising with the multidisciplinary team to facilitate the development of effective discharge packages and to ensure that appropriate care plans are implemented for ongoing management.
• Providing specialist advice and support for all health professionals, patients, carers and other agencies.
• Developing, implementing and evaluating education programmes for primary care staff on the effective management of Parkinson’s.
• Facilitating the teaching of patients, families and carers in the understanding and management of Parkinson’s.
• By acting as a resource for all professional development programmes related to Parkinson’s Disease.

Who can be referred?
People who have been diagnosed by a Consultant Neurologist or Consultant Physician with a Special Interest in:-

• Idiopathic Parkinson’s Disease
• Cerebrovascular Parkinson’s Disease
• Parkinson’s Plus Syndrome
• Parkinsonism
• Age 16 and over.
• Registered with a GP within the area of Bassetlaw.

Who can make a referral?
• The patient or carer/relative with the patient’s consent.
• Any health or social care professional with the consent of the patient.
• NB Patient’s unable to give consent will be treated at the discretion of the Nurse Specialist if involvement is in the best interest of the patient.
