Who are we?
We are highly experienced Health Care Support Workers, providing nursing care and support to patients and their carers within the patient’s home. The service is available during the day and at night time providing respite care for carers. The sitters are co-ordinated by the Palliative Care Co-ordinator.
Who do we work with?
Patients who are registered with a Bassetlaw GP and who have had a full assessment from a referring professional.
The service provides help and support for carers and relative of patients with a long-term condition or a cancer diagnosis, who are entering the last months or weeks of life.
Who is in the team?
Eight full time and part time Health Care Support Workers who work on a rotational basis.
How can we help?
We will provide respite care either during the daytime or at night time. Day sits are generally mornings or afternoon, and night sits commence at 10pm until 7am the following morning. The aim of the service is to provide ongoing support to carers and where appropriate facilitate early discharge from hospital.
How will you benefit?
The respite visits enable the carer to take some time out from their caring responsibilities for shopping, socialising or just to have a break from caring. The night sits enable the carer to have a good nights sleep, safe in the knowledge that the person they are caring for is well looked after, and that their personal care needs will be attended to.
The Palliative Care Coordinator will endeavour to meet individual requirements when arranging night sits and day sits, wherever possible, and will liaise directly with families regarding the care available on a weekly basis.
Who can make a referral?
Referrals are accepted from District Nurses, GP’s, Hospice, Macmillan Nurses, Social Workers and other health care professionals.
Referrals are made to the Palliative Care Coordinator on 01777 863352 between 9.00am and 5.00pm, Monday to Friday and at weekends and Bank Holidays via Single Point of Access at Retford Hospital on 01777 274422 between 7.00am and 9.00pm.
