Tissue Viability Service

Who are we?
The Tissue Viability Service provides a specialist providing advice and/or education to patients, carers and healthcare professionals within the community setting on all aspects of wound care management, pressure ulcer prevention and management and leg ulcer management.  The service aims to maintain and promote clinical excellence within the care of wounds and pressure ulcers.

Who is in the Team?
The service is staffed by an experienced Tissue Viability Specialist.

How can we help?
Following a referral to the service an assessment is made by the nurse specialist and a management plan is put into place supported by advice and specific patient education as appropriate.

 
Who can make a referral?
Referrals can be made from any health professional via the Single Point of Access on the following numbers.  Messages will be forwarded to the Tissue Viability Service.

Who can access the service?
Patients who have a Bassetlaw GP, community and Primary care staff.
