The Directorate has two multi disciplinary community teams, one based in the City and a second in the County. They are responsible for supporting and supervising mentally disordered offenders living in the community, many of whom have been discharged from prisons or transferred from secure hospitals. The teams work in partnership with criminal justice agencies in court diversion, court liaison, the police service and the provision of consultation and support to colleagues in Social Services, Health and the Probation Service.
The community teams also provide through-care for Nottinghamshire community forensic service patients placed in psychiatric units outside of the Trust's services, maintaining a vital link and preparing for their eventual return home.
