Community Mental Health Teams (CMHT) for older people are specialist teams that provide community based assessment and treatment for people of any age suffering from dementia (organic) and those people over the age of 65 years who are experiencing mental health problems (functional).
Each team has a multi-disciplinary mix of staff including psychiatric nurses, occupational therapists, consultant psychiatrists, social workers, physiotherapists, cognitive behavioural psychotherapists and psychologists. The teams are able to offer a comprehensive service to people in their own homes and act as gate keepers to other services which assist people to maximise their quality of life in the community. We also have a specialist dementia outreach team who visit nursing and residential homes to provide education, support and treatment to the care home staff and also the residents within the home who are suffering from dementia.

Each team has a single point of access for referrals.
Day care services are provided throughout Nottinghamshire, ensuring people have access to assessment and treatment in a day care environment. It provides an opportunity to offer a comprehensive assessment of need and structured treatment options to individuals.
