Therapy Team
The therapy team includes specialist physiotherapists and occupational therapists directly linking in to community, inpatients and day care areas within mental health services for older people. They provide assessment and treatments in areas such as, balance, falls risk, improving mobility, personal care, domestic and leisure activities, cognition, communication and mood. The therapy team works with service users and their carers to promote a sense of independence and quality of life.
Acute Care Liaison Team
Specialist team offering assessment and treatment to older people with mental health problems and people suffering from dementia who are in a general hospital setting. The team is currently based at Kingsmill Hospital and also covers Mansfield Community Hospital and Ashfield Community Hospital. The team offers training packages to staff that work in these hospitals to improve their skills and level of expertise in this area of nursing.
Outreach Team
This team provides specialist staff to support older people with dementia and their carers in residential or nursing homes during times of crisis or difficulty. It also provides training packages to staff in care homes in order to develop skills, expertise and knowledge in caring for people with dementia. The team is currently based in Nottingham City South.
Hospital Discharge Facilitator
Discharges from hospital normally follow a well rehearsed pattern. However, occasionally this process can become more complicated for a variety of reasons. The Mental Health Services for Older people directorate employs a discharge facilitator to deal with these more complex discharges. The ward staff will normally ask the discharge facilitator to get involved if they feel that there is a need for more skilled handling of the discharge process. However, anyone can request the input of the discharge facilitator via any member of the ward team.
