Community Macmillan Team
The Community Macmillan Team aim to provide holistic specialist end of life care for patients with a non-curable diagnosis. We work closely with your GP to offer specialist symptom control, advanced care planning and psychological support to patients and their families in a variety of community settings.
We provide educational courses for non-qualified and qualified health care professionals focusing on end of life care, advanced communication skills and physical examination skills.
Our team work closely with Nottinghamshire Hospice and Hayward House who provide day therapy and in-patient care to support our patients and families throughout their illness.
This service is available to people over 18 years of age, thought to be in their last year of life with a Nottinghamshire County GP.
