Complex Health Needs Support Team

We support children and young people with complex health needs, so that they can get their education at school.

This service is for children and young people who are on role at Oak Field School and Sports College and who are identified through the continuing care process as needing continuous health support in school in order to access education.
 

Service opening hours

8:30am – 4pm Monday to Friday. Term time only.

 

Accessing our service

To access this service you will need a continuing care assessment to be undertaken. This needs to be discussed with the class teacher and the school nurse.

Getting here

Visitor parking is available on site. Some spaces are only for blue badge holders. Oak Field is on bus route number 28 from the city centre.
