Chronic Obstructive Pulmonary Disease (COPD) Community Services provide case management, including integrated, multi-disciplinary assessment and treatment through person-centred care plans, in line with case management strategies.
Pulmonary rehabilitation programmes consist of exercise and education to help patients improve their quality of life by monitoring and managing their condition. These are delivered in each of the localities and patients offered a choice of venues.
