How to refer someone to this service

Referrals to the Children's Community Nursing team are made direct from community paediatricians based within the Children's Development Centre (South County and City only).

Children's clinics at the Children's Development Centre:

All referrals to the Children's Community Nursing Team in Mid-Notts should go through the Single Point of Access (SPA) on 0300 123 3387.

Short Breaks (respite care):
All referrals for overnight Short Breaks must be made via the child/young person's social worker.

Contact the relevant disabled children's team in the City or County.

Packages of care for short breaks are agreed at either County or City interagency resource meetings.

Following a referral from social care for the service a health assessment will be undertaken by an experienced health professional, who has current knowledge of the child/young person's health needs, usually the child's school/community nurse, or nurse from the short breaks unit.

Professionals are welcome to contact the service to arrange an insight visit. Call 0115 883 1147 to arrange.

Referrals for day care short breaks can be made by any health professional via a Common Assessment Framework (CAF) or following a Team Around the Child (TAC) meeting, directly to the service.
