Nottingham City Specialist Dementia Outreach Team
We work to ensure the improved quality of care for people with dementia in care homes in Nottingham City by providing specialist assessment and support.
The service focuses on the needs of individual residents, looking at ways to enhance mental wellbeing. The team works with care home staff to develop their knowledge, skills and confidence to plan and deliver appropriate and individualised dementia care.

Accessing our services
Referrals are accepted only from the patient's GP, Mental Health Services for Older People inpatient wards or the Continuing Care Team.
Patients are seen in the care home.
