Primary Care Rehabilitation

We provide rehabilitation in patients’ own homes.

We give patients the skills to help themselves. This allows them to keep their independence and improve their physical function.

Patients have an assessment with the team’s occupational therapist and/or physiotherapist. They will then have personalised support, including:

  • exercise to improve strength and balance
  • home hazard assessments
  • specialist equipment
  • strategies to reduce the risk of falling, if they have a history of falls

Our service is available to all patients over the age of 18 who are registered with a GP in:

Monday to Friday from 8:30am – 4:30pm.

 

Accessing our service

If you think you need our service, please call the Adult Community Services referral hub on 0300 0830100, between 8am and 5pm. If our service is right for you, we will arrange an appointment to visit you at home.  

Health professionals can also refer people using the number above.

 

Information for health professionals

Referrals are welcomed from GPs, community health and social services, nursing and residential care homes, patients and carers and voluntary sector colleagues. Please call the Adult Community Services referral hub on 0300 0830100, between 8am and 5pm.
