This information pack is for use by staff in the Memory Assessment Service (MAS) completing the first clinic appointment with patients/carers who are experiencing memory problems and may need further tests for diagnosis.
It includes information that is given to you at your first clinic appointment with the Memory Assessment Service. You may also be given additional information specific to you, which your healthcare professional may have discussed with you during/following the appointment.
