Memory Assessment Service: second clinic appointment pack (Diagnosis and Support Information)

This information pack is for use by staff in the Memory Assessment Service (MAS) completing the second clinic appointment with patients/carers who have had tests and returned to the clinic for diagnosis of their memory problems.

It includes information that is given to you at your second clinic appointment with the Memory Assessment Service. You may also be given additional information specific to you, which your healthcare professional may have discussed with you during/following the appointment.
