Preparing for Adulthood and Transition to Adult Health Services

At the Community Children and Young People’s Specialist service we want to support young people and families who are starting the journey through adolescence as they move from Children’s to Adult care. This is called Preparation for Adulthood or Transition.

Our Services provide specialist Nursing, Occupational Therapy, Speech and Language therapy and Physiotherapy services across Nottinghamshire.

This information is for all young people and families using those services, but general Information may be helpful for those who are not. 

This information will also signpost to national, Nottingham City and Nottinghamshire County services.
