We offer a range of heart failure services, including Community Heart Failure Specialist Nurse Service and South Nottinghamshire Heart Failure Specialist Nurses.
Community Heart Failure Specialist Nurse
Our Heart Failure Specialist Nurses support people who are living with heart failure in the community.
We work closely with GPs, hospital teams and other health professionals to make sure people get the right care at the right time.
The service aims to:
- help people live as well as possible with heart failure
- reduce emergency hospital admissions
- support people to make choices about their future and end of life care

South Nottinghamshire Heart Failure Specialist Nurses
The South Nottinghamshire Heart Failure Specialist Nurses provide specialist care for people with heart failure in their local community.
We use up-to-date clinical guidance and work with primary and secondary care teams to improve quality of life, prevent avoidable hospital stays and support end of life planning when needed.
