These Easy Read leaflets are designed to help service users and patients understand important health information.

The leaflets are not intended to replace a conversation with the service user or patient and should always be shared as part of a discussion.

It is important that the information is properly explained and understood.

A member of staff, a support worker, a relative, or a parent should go through the leaflet with the service user or patient.
The person providing support should clearly explain what the words and pictures mean.
They should check that the service user or patient understands the information being shared.
