Registration for ages under 18

Guidance to completing the Young Person’s Registration form.

01 Personal details

We ask the questions about you to ensure that we have the correct information about you, for example, the name you like to be known as (or called).

We ask for your contact details so that we can contact you about other pieces of work you may be interested in getting involved with. This will include things from other services and organisations.

02 Getting to know you

Do you need anything to help you feel safe? - we would like you to share with us anything that you might need to enable you to jointhe meeting or work which will impact you positively. This could include things like:

  • Not having the room to hot
  • Letting me speak in my own time without any pressure or beingput on the spot.
  • Having paper and pens available to me.

We may not always be in a position where we can do everything, but we will support you as much as possible.

When we ask about caring for someone, we’d like to know if you are there for someone who is experiencing some difficulties in their life and you’re that person they can speak to, or maybe it’s alittle more than that, maybe you support in more practical ways butwhat we’re saying is, are you there when someone you love needs you? If that’s a yes, then give this box a tick.

We ask you this so we can offer you support if needed but also it might open other opportunities for you to become involved with.

03 Parent or Legal Guardian

We need information about the adults who care for you so that we can contact them to let them know we are storing your details. We also need to make sure that they are aware of the work you are getting involved with and that they are ok with it. We also ask for this infroamtion so that in the case of an emergency we can get hold of the people you would want us to speak to.

04 Demographic Information

Questions about gender, ethnic group and disabilities  are all classed as demographic information. We collect this information so that we can look closer at who is engaging with us and if there are areas of people that we are not hearing from. We want to make sure that we are working with as many people as possible.

05 Data Protection

All of the information we ask for we store securely on a data base that only staff members with specific access can see, these staff members include members of the Involvement Experience and Volunteering team, and staff members you are working with on specific work, meetings and/or projects.

We want people with experience of our services to have the opportunity to share their stories with us and influence the decisions we make. We try to involve as many people as possible in the work we do to improve care and services. 

Please complete our registration form to let us know who you are and how you’d like to be involved. Once you have completed this form we will confirm your registration by email and you will be sent information about the upcoming project or role that you have asked to be involved in along with any training opportunities. You will also be given the name and contact details of the host of the project with which you're involved in.
