Current Bassetlaw LD Nursing Service Overview

Children and young people (CYP) aged 0-24 with moderate to severe learning disabilities, including those with Autism, Down's Syndrome, ADHD, and significant developmental delay, registered with a Bassetlaw GP or resident in Bassetlaw. The service aims to:

  • Enable CYP and families to access, participate, and contribute within their communities.
  • Promote social inclusion, positive physical, mental, and emotional health outcomes.
  • Prevent placement breakdown at home, school, or other settings.
  • Provide early intervention to avoid crisis escalation

Sleep Support – What’s Available and How to Access

Sleep Charity and Sleep Tight Programme

  • The Sleep Charity is commissioned by both Nottinghamshire County Council (NCC) and the Neurodevelopmental Support Service (NST). They provide training and accreditation for staff to deliver the Sleep Tight programme.
  • Sleep Tight in Family Hubs

 

Who for

Parents of children aged 1-4 with poor sleep patterns due to environmental factors, including children with additional needs.

 

Aims

  • Understand why a child may have sleep difficulties
  • Learn about sleep cycles and good bedtime environments
  • Develop bedtime routines
  • Address sleep deprivation, night waking's, anxiety, and self-settling
  • Use sleep diaries, checklists, and reward systems

 

Format

5 weekly sessions, 1.5-2 hours each, for groups of 4-10 parents.

Neurodevelopmental Pathway Bassetlaw (NPB)

Access to services for children and young people with neurodevelopmental differences such as Autism and ADHD. It brings together multiple stakeholders, including Community Paediatrics, CAMHS, education services, and voluntary sector partners to ensure a coordinated, child-centred approach to assessment, diagnosis, and support.

APTCOO is a registered charity and independent special school that provides a wide range of specialist support services for children and young people with Special Educational Needs and Disabilities (SEND), including those affected by early life trauma.

Healthy Families Team

Does not provide sleep courses or groups but will support families as needed and typically refers to Family Hubs or Neurodevelopmental Support Team for structured sleep support.

Keyworking Service

The Autism and Learning Disability Keyworking Service is a vital support initiative for children and young people aged 0-25 with a diagnosis of autism or a learning disability, particularly those at risk of hospital admission or in need of enhanced community support.

  • Prevent crisis and escalation by intervening early and tailoring support across health, education, and social care.
  • Coordinate care to reduce stress and uncertainty for families.
  • Advocate for children and families, ensuring their voices are heard and their needs are central to planning.
  • Support transitions between home, school, community, and hospital settings.

Dynamic Support Register (DSR)

The Dynamic Support Register (DSR) is a nationally mandated, locally implemented system that helps health and care professionals identify individuals with a diagnosis of learning disabilities and/or autism who are at risk of crisis, particularly those at risk of hospital admission. It enables a multi-agency, proactive approach to care planning and crisis prevention. 

Individuals may be added to the DSR if they:

  • Are at risk of mental health hospitalisation.
  • Present significant risk to themselves or others.
  • Are experiencing placement breakdown (e.g. residential school or supported living).
  • Have recently been discharged from a mental health hospital.
  • Have been supported by a crisis team in the past year.
  • Are presenting to emergency services due to mental health concerns.
  • Have a diagnosis of autism and/or learning disabilities.

Dynamic Support Register

Behaviour Intensive Community Support (BICS)

The Behaviourally Intensive Community Support (BICS) service in Nottinghamshire is a specialist behavioural support programme commissioned by the NHS Nottingham and Nottinghamshire ICB and delivered by the Positive Support Group (PSG). It is designed to support children and young people with autism and/or learning disabilities who are at risk of placement breakdown or hospital admission due to behaviours of concern.

The BICS service provides intensive, community-based behavioural support to:

  • Children and young people aged 0-18 with a formal diagnosis of autism and/or a learning disability.
  • Individuals at risk of family or placement breakdown, or hospital admission.
  • Those referred through the Dynamic Support Register (DSR).

Continence Support

Healthy Families Programme (HFP) and Community Nursing

 

Offer

  • Integrated support between HFP Public Health Nurses and community nurses (part of CCYPS):
  • Advice, information, and awareness on nocturnal enuresis, constipation, and toilet training problems.
  • Support for healthy bladders and bowels, prevention and management of constipation.
  • Interventions for toilet training, review of input/output charts, first-line treatments (e.g., toileting/fluid/diet advice, enuresis alarms).

 

Age Range

Generally, for school-aged children (5-11), with pre-school support mainly via Family Hubs/Children's Centres.

 

Bassetlaw Specifics

CCYPS does not currently deliver in Bassetlaw due to legacy commissioning; HFP team likely supports all children but check for any alternative local offers. For further clarification, contact the Public Health and Commissioning Manager at Nottinghamshire County Council.

 

Note:

  • If under 5 with no additional needs, remain with HFT, if over 4 with additional needs, continence team involvement.
  • Behavioural continence issues: not supported, signposting to ERIC or Bladder and Bowel UK and resource - Toilet Training and the Autistic Spectrum (Amazon) for smearing.

CAMHS Intellectual Disability Team

Supports children and young people who have a moderate to severe intellectual disability and mental health needs. Typically, this is a young person who attends a special school with an IQ of 55 and below.

The team works with their parents and carers, schools, health professionals, social care and short break providers, and offer tailored advice and strategies for each young person.

Their main base is at 68 Portland Street, Kirkby in Ashfield and we also hold clinics across the county and city.
