Only visit your local Emergency Department for serious life-threatening conditions that need immediate medical attention including persistent severe chest pain, loss of consciousness, acute confused state, severe breathlessness, severe blood loss, serious burns or suspected stroke.
Our service is based at the Horizon Day Assessment and Treatment Unit. We provide longer-term day treatment for adults with intellectual disabilities and significant levels of need.
We also provide short-term placements for people who have intellectual disabilities and complex needs that cannot be assessed or treated within mainstream or community health settings.
Complex needs involve severely challenging behaviour, mental health problems, or significant health needs that require specialist investigation such as epilepsy, disorders of appetite, or dementia.
Our service is available to those over 18. There is disabled access to the Horizon Unit. Car parking is available.

Outside of office hours please contact your GP service, NHS 111, or in an emergency attend your local Accident and Emergency (A&E) department.
The referral form is accessible via the intranet if you are part of the Trust. A copy of the referral form can be sent via post, fax or email as requested, alternatively call Horizon and we can discuss the referral process.
