The Heart Failure Nursing Services provide a community focused specialist service to support people living with heart failure.
The aims of these specialist services are to work in partnership with primary and secondary care health professionals, using evidence based guidelines, to improve quality of life, reduce unplanned hospital admissions and facilitate end of life choices among heart failure patients.
