Celebrating Community Service Week

As we mark Community Services Week, we celebrate the dedication and hard work of our community care teams. From nursing in homes to rehabilitation after illness, from caring for our youngest residents to supporting the elderly in their final stages of life, our community services are an essential pillar of health and well-being in Nottinghamshire.
Hosted by the NHS Confederation and NHS Providers, the week brings community services to life and celebrates the successes and the impact this has on people's lives.
This year, with the Ten-Year Health Plan putting the establishment of a Neighbourhood Health Service front and centre of government policy, there has never been a better time to put community services in the spotlight.
Join us in recognising the outstanding contribution our community services make to keeping local communities healthy, cared for, and supported.
We want to also highlight the integral role played by our community services in Nottinghamshire and beyond.
At Nottinghamshire Healthcare, our Community Health and Specialist Services Care Group provides a wide range of physical and specialist healthcare across Nottinghamshire, as well as some residents in neighbouring areas such as Derbyshire and Leicestershire. We also work in partnership with organisations including Nottinghamshire County Council, Nottingham University Hospitals and Sherwood Forest Hospitals to deliver integrated, high-quality care.
Our services are organised into four specialist units:
Urgent Care Unit - Delivering rapid response and rehabilitation services such as Lings Bar Hospital inpatient care, urgent community response, virtual wards, discharge-to-assess pathways, community therapy, podiatry, MSK services, alcohol and long-term conditions support, nutrition and dietetics, and speech and language therapy.
Planned and Proactive Care Unit - Providing ongoing and preventative care including adult community nursing, cardiopulmonary services, heart failure care, neurological rehabilitation, stroke services, respiratory support, diabetes care, end-of-life care (including John Eastwood and Cedar House hospices), Macmillan nurse services, and post/long COVID care.
Specialist Care Unit - Offering targeted specialist provision including learning disability services, intellectual disability support, the Nottingham Centre for Transgender Health, neuropsychology, day services, and intensive assessment and treatment for complex needs.
Offender Health Care Unit - Delivering integrated healthcare across several prisons, including HMP Nottingham, HMP Ranby, HMP Lincoln, HMP Morton Hall, HMP Swinderby, HMP North Sea Camp, and Offender Personality Disorder (OPD) services.
The majority of our services are delivered in people's own homes or local community settings, with some specialist and inpatient services provided at dedicated facilities. By working closely with primary care, hospital partners and the voluntary sector, we ensure people receive the right care, in the right place, at the right time.
