Stacey Birley-Tedder, Primary Care Liaison Nurse, Primary Care Liaison Team at the Trust was recently awarded a national Cavell Star Award, as well as a Trust award. This was in recognition of Stacey's work with a vulnerable patient for the last two years, providing outstanding care and support.

An inspiring national awards programme, Cavell Star Awards are given to nurses, midwives, nursing associates and healthcare assistants who shine bright and show exceptional care to one of three groups of people, their colleagues, their patients or patients' families

She was also awarded a Positive Stars Award, which is the Trust's monthly colleague recognition scheme and was surprised with both on the same day.

The patient Stacey was supporting wasn't attending annual health checks and there were also safeguarding concerns Stacey raised after visiting his home.

The patient was found to be not collecting his repeat prescription of crucial medication. She liaised with the pharmacy and alerts were added to their system so if he was not collecting prescriptions this would be flagged. Stacey also supported him to collect his medications, so he knew where the pharmacy was and was able to go himself.

The patient had also not seen a dentist or optician in many years, and she was able to organise this for him and went to the appointments with him.

Amy Eagle, Care Group Director of Community Health and Specialist Services at Nottinghamshire Healthcare said:

We are incredibly proud of Stacey for being recognised both nationally and within the Trust for the exceptional care she provided to this patient. She consistently went above and beyond to ensure he received the support he needed, demonstrating genuine compassion, dedication, and an unwavering commitment to his wellbeing. This recognition is richly deserved - huge congratulations to Stacey on this outstanding achievement.

She identified he needed a package of care which would support him in all aspects of his life including collecting medication, co-ordinating and attending appointments.

Stacey referred to social care and advocated strongly for this package of care and ensured it was in place.

The patient's trust in Stacey and her perseverance with supporting him has helped him consider other options and he is moving into supported living in the near future.

One of Stacey's nominators for both awards, Lucy Miller, Primary Care Liaison Nurse said:She has touched all aspects of the patient's life, gained his trust and without her support his life could have taken an even more negative turn and without exaggeration could have led to the direst of consequences. Her dedicated, constant and determined support demonstrates all the trust values and more and as a team we are so proud of the difference she has made.
