Young Peoples Transition Pathway

Transitioning from Child and Adolescent Mental Health Services (CAMHS) to Adult Mental Health Services can feel like a big step, but you're not alone. We're here to support you every step of the way. 

As you move into adult services, you'll continue to receive the care and support you need, tailored to your individual needs. Our team will work closely with you to make this transition as smooth as possible, ensuring that your voice is heard, and your concerns are addressed. 

Whether it's understanding your new care plan or getting to know your new team, we're committed to helping you feel confident and supported throughout this process.

 

Our 10 step pathway


Step 1: 14 years + Identify


We identify young people needing transition support.

Some young people may start there transition journey as young as 14, this will happen if you have a educational health care plan please follow this link: Preparing for Adulthood and Transition to Adult Health Services.

If you do not your transition support from the transition hub / service will begin at 17.

But your CAMHS team will start to:

 

Step 2: Empower Self-Care


We empower you to develop self-care skills, helping you take an active role in your wellbeing.

Whilst you are under CAMHS from age 14+ your worker will empower you to develop self-care skills, helping you take an active role in your wellbeing. From the age of 16 you have the same rights as an adult when it comes to consent for treatment and confidentiality.

You may have this right sooner under Gillick competence.

 

Step 3: Start Transition Plan


Together, we create a personalised transition plan tailored to your needs.

You may have a structured plan or a plan for transition within your CAMHS care plan.

Step 4: Review Multidisciplinary Team


We review your care team to ensure all necessary support is in place.

 

Step 5: Refer to Adult Services


With your full agreement your CAMHS team or an allocated worker from the transition hub / service will refer you to the appropriate adult service or your GP, ensuring a clear pathway forward.

Step 6: Joint Review (Children's Services Lead)


Children's services lead a joint review to align your care with adult services.

Once an adult service has been identified CAMHS services should organise a review with your self and a carer or family member if you wish and the adult service you will be going to.

You can ask CAMHS to do this in your absence, but it is better if you meet with the new worker and service provider in good time before you are closed to CAMHS to develop a relationship.

 

Step 7: Plan Urgent Care Route


We plan a route into urgent care, so you know where to turn in a crisis.

Pre 18 you will receive crisis support from CAMHS crisis, at 18 this will come from adult crisis your team will inform you of which team or duty will support you and how to contact them.

Step 8: Confidence to Transition


The transition hub / service or your CAMHS team will work with you until you feel confident and ready to move to adult services.

 

Step 9: Joint Reviews (Adult Services Lead)


Adult services take the lead in joint reviews as you prepare for transition.

This is the time when you will discuss whether or not if you are ready to say goodbye to your CAMHS worker and move over to adult to work with your new adult service.

 

Step 10: Settle into Adult Services


Finally, we ensure you are settled into adult services with all the support you need.

The transition hub may contact you directly or send you out an evaluation form of how your experience of transition was and if there are any changes you could suggest to improve this experience.
