Only visit your local Emergency Department for serious life-threatening conditions that need immediate medical attention including persistent severe chest pain, loss of consciousness, acute confused state, severe breathlessness, severe blood loss, serious burns or suspected stroke.
We work with all providers of diabetes care – such as hospitals, GPs, and care homes - to support patients with type 2 diabetes to manage their condition. We offer this service in Bassetlaw.
We offer expert advice and support to GP practices treating adults with type 2 diabetes who are unstable or have complex needs. The diabetes specialist nurses see patients alongside the nurse or GP. They also visit housebound patients on a short term basis.
Our service is for people over 18 years of age. This does not include patients with type 1 diabetes.
This service is available to those registered with a GP in the Bassetlaw area. Access to this service is via GP referral only, district nurse, community matron or hospital consultant.
