Community Diabetes Nursing Service - Bassetlaw
Our service is designed to support patients with type 2 diabetes to manage their condition. We offer this service in Bassetlaw to patients aged over 18.
We work with all providers of diabetes care such as hospitals, GPs and care homes. Our expert advice and support can help GP practices to treat adults with type 2 diabetes who are unstable or have complex needs. Our diabetes specialist nurses see patients alongside the nurse or GP and can also visit housebound patients on a short-term basis.
Our service is not for patients with type 1 diabetes.
Accessing our service
This service is available to those registered with a GP in the Bassetlaw area. Access to this service is via GP, district nurse, community matron or hospital consultant referral only.
