Community Diabetes Nursing Service - Bassetlaw

We support adults with type 2 diabetes to manage their condition. 

This service is available in Bassetlaw for people aged 18 and over. 

We work with all diabetes care providers, including: 

  • GP practices
  • Hospitals
  • Care homes 

Our team offers expert advice and support to GP practices. We help care for adults with type 2 diabetes who are not well controlled or who have complex health needs. 
Our diabetes specialist nurses can: 

  • See patients with the GP or practice nurse
  • Visit patients at home for a short time if they are housebound

This service is not for people with type 1 diabetes.

Accessing our service

This service is available to those registered with a GP in the Bassetlaw area. Access to this service is via GP, district nurse, community matron or hospital consultant referral only.
