John Eastwood Hospice

John Eastwood Hospice is a purpose-built, 12-bedded specialist palliative care unit. We are part of the End of Life Care Together programme which serves people across Mid Nottinghamshire.

Accessing Palliative and End of Life Services

The End of Life Care Together service for Mid-Nottinghamshire provides:

  • Clinical triage, assessment and coordination of patient and carer needs
  • Hospice at Home services, including a dedicated nursing service to provide care in the home through the night and during the day dependent on the Patient or Carers needs (provided by Beaumond House Community Hospice and Notts Hospice)
  • Community Hospice beds, at Beaumond House Community Hospice
  • Specialist Palliative Care inpatient services at John Eastwood Hospice
  • Hospital in reach in partnership Sherwood Forest Hospital NHS Trust
  • Outreach by Specialist Palliative Nurses by John Eastwood Hospice and Primary Integrated Community Services Ltd, to support patients and carers in their home
  • Day Hospice from Beaumond House Community Hospice and through the Living Well Centre at John Eastwood Hospice
  • Bereavement services, Clinical Psychology services and access to carers support services and groups

All referrals can be made via a single point of referral - Call for Care 01623 781899 option 2,

Referrals can be made during working hours 7 day a week service. Clinical advice is available via this same number 24hrs each day.

South Nottinghamshire Community Macmillan Team

The South Nottinghamshire Community Macmillan Team aim to provide holistic specialist end of life care for patients with a non-curable diagnosis. We work closely with your GP to offer specialist symptom control, advanced care planning and psychological support to patients and their families in a variety of community settings.

We provide educational courses for non-qualified and qualified health care professionals focusing on end of life care, advanced communication skills and physical examination skills.

Our team work closely with Nottinghamshire Hospice and Hayward House who provide day therapy and in-patient care to support our patients and families throughout their illness.

This service is available to people over 18 years of age, thought to be in their last year of life with a Nottinghamshire County GP.

 

Accessing our Services

Referrals can be made by GP, district nurse, clinical nurse specialist or health professional within the acute hospital trusts.

Service Opening Times

Our Community Macmillan Team work Monday – Friday between the hours of 8:00am – 5:00pm. If you require support out of hours, please contact your local District Nursing Team on the following number: 0115 9133000.
