John Eastwood Hospice is a purpose built, 12 bedded specialist palliative care unit providing support. The Hospice is part of the Mid Notts ‘End of Life Care Together’ delivery model serving the population of Mid Nottinghamshire.

The South Nottinghamshire Community Macmillan Team aim to provide holistic specialist end of life care for patients with a non-curable diagnosis. We work closely with your GP to offer specialist symptom control, advanced care planning and psychological support to patients and their families in a variety of community settings.
We provide educational courses for non-qualified and qualified health care professionals focusing on end of life care, advanced communication skills and physical examination skills.
Our team work closely with Nottinghamshire Hospice and Hayward House who provide day therapy and in-patient care to support our patients and families throughout their illness.
This service is available to people over 18 years of age, thought to be in their last year of life with a Nottinghamshire County GP.
Referrals can be made by GP, district nurse, clinical nurse specialist or health professional within the acute hospital trusts.

Our Community Macmillan Team work Monday – Friday between the hours of 8:00am – 5:00pm. If you require support out of hours, please contact your local District Nursing Team on the following number: 0115 9133000.
