How to refer

Anyone with a GP within Nottinghamshire County can self-refer into the service.

For a referral to be considered, patients / individuals must present with persistent long / post covid symptoms for longer than 12 weeks following initial infection. A positive covid test is not required to be accepted by the service.

Any other health-related issues that cause similar symptoms require exclusion by the GP prior to referral.

Referrals can also be made by any GP within Nottinghamshire County via F12 or on the Post Covid Referral form.

Patients with a GP within Nottinghamshire County can access the service.

We are introducing online appointment booking.

If we have received your referral and you have agreed to digital contact, we will send you a booking link by text message.

Using the booking link

You will need to enter your date of birth to access the system.
Appointments are shown using a date drop down menu.
If you change the date range, you must press Search to update the list.
Your first appointment will be a telephone assessment. The system may show an address for Trust Headquarters. This is normal. Your appointment will still be by telephone. Please do not travel to this appointment.
After your telephone assessment, you may be offered a clinic appointment. If agreed, you will receive another booking link by text message.
Clinic locations are shown in a location drop down menu. This will only show locations available in your chosen date range. Press Search to update your selection.
You can choose an appointment close to where you live, if available. If you need an earlier appointment, you may choose a different location. This does not mean you must attend the same location again.
Please book your appointment as soon as possible. Booking links will expire.

I had been struggling with my symptoms for a while and was referred to a number of hospital departments. It was difficult to make sense of my symptoms and how they linked together.

Then my GP referred me to the Long Covid Service and within weeks I'd had my first assessment meeting. Immediately, I felt like I wasn't going crazy and that all the symptoms I had were linked to Long Covid. It felt affirming to finally find professionals who saw me as a whole.

From then, every professional from the Team has been amazing! From the comprehensive symptom management sessions, to specialist practitioners who have offered me detailed and practical advice, support, and equipment, I cannot say how life changing the Team has been.

With the amazing support I have received, I really wanted to be involved in the ongoing development of the service. I have been delighted to be able to give something back as part of the involvement group. These sessions are a great opportunity to meet other people in similar circumstances in a structured, positive and supported way. This suits me well and I look forward to attending each month!
