As you progress through your phased return, it is recommended that you keep in regular contact with your manager. Setting aside time to review how things are progressing can be a great way of keeping on track and ensuring that you remain as well as possible in, and out, of the workplace. It can help to consider the following points before you meet with your manager. It might also be useful to share this document with your manager.

The focus is shifting from reactive management of sickness absence to a more proactive approach of prevention through promoting wellbeing and improving employee engagement.

Reasonable adjustments disability passports (TUC and GMB)
A reasonable adjustments passport is a live record of adjustments agreed between a worker and their manager to support them at work because of a health condition, impairment or disability.
The passport supports members and their reps to work with employers to ensure that up-to-date, appropriate, practical and correct reasonable adjustments are in place.
Work Rehabilitation (from home) (University Hospitals of Derby and Burton NHS Foundation Trust)
Evidence suggests that work is generally good for health. As well as financial security, it provides us with routine and a sense of identity. Simulating the work environment at home can be a good way for you to prepare for a phased return. It will allow you to develop symptom management strategies and establish comfortable limits in work-related tasks. However, if you are too unwell to consider a phased return, it can still be a great way to add structure to your day and manage 'illness work'.
