Only visit your local Emergency Department for serious life-threatening conditions that need immediate medical attention including persistent severe chest pain, loss of consciousness, acute confused state, severe breathlessness, severe blood loss, serious burns or suspected stroke.
This service covers Larwood and Bawtry, Retford and villages, and Newgate Street primary care network teams.
The Integrated Teams work with people with long term conditions and are wrapped around GP Practices. They include professionals such as:
All of who link with other specialist services e.g. Tissue Viability Nurses and Continence Specialist Nurses.
Each neighbourhood team has an integrated team leader and a ward co-ordinator who will provide a first line of contact and ensure the smooth running of the teams. The teams can be contacted via Single Point of Referral (SPR) on 01777 274 422.
The core District Nursing Service operates 24 hours a day, incoporating the twilight and out of hours service.
