Primary Care Networks - Bassetlaw
Our service covers the Larwood and Bawtry, Retford and villages, and Newgate Street areas.
Our integrated teams work with people who have long-term health conditions. They work closely with local GP practices.
The teams include:
- District Nurses, Staff Nurses, Nurse Associates and Healthcare Support Workers
- Phlebotomists (staff who take blood samples)
- Occupational Therapists, Physiotherapists and Assistant Practitioners
- Falls Specialists
- Ward Co-ordinators
Team members also work with other specialist services when needed. These include Tissue Viability Nurses and Continence Specialist Nurses.
Accessing our services
Each neighbourhood team has an integrated team leader and a ward co-ordinator who will provide a first line of contact and ensure the smooth running of the teams. The teams can be contacted via Single Point of Referral (SPR) on 01777 274 422.
Service opening hours
The core District Nursing Service operates 24 hours a day, incoporating the twilight and out of hours service.
