RECONNECT

RECONNECT offers short term support to people leaving prison. 

This includes people: 

  • leaving prison in Nottingham or Nottinghamshire, or
  • returning to Nottingham City or Nottinghamshire from prisons elsewhere in England. 

RECONNECT helps people reconnect with healthcare services in the community. 

We do not provide medical treatment and cannot replace existing care providers. 

RECONNECT also supports people returning to other areas in England through a cross border pathway. 

Please speak to the RECONNECT team before making a referral for this type of support.

The RECONNECT team 

The team includes: 

  • a Team Leader
  • RECONNECT Practitioners
  • Peer Support Workers
  • Community Support Workers  

Who can RECONNECT support? 

To be eligible for Nottinghamshire RECONNECT, the person must: 

  • be in prison, and
  • be returning to Nottingham City or Nottinghamshire.

They must also:

  • have a health need, and
  • be willing to engage with services.

Health needs can include:

  • physical health needs
  • mental health needs
  • neurodevelopmental conditions
  • substance misuse
  • complex or multiple needs

The person may struggle with resettlement because of issues such as: 

  • difficulty looking after their health and wellbeing
  • problems managing appointments
  • chaotic or unsettled behaviour

What support can RECONNECT offer?

Once a referral is received, RECONNECT staff will complete a full needs assessment. 

Following the assessment, a decision will be made about whether the person can be taken onto the caseload.

If accepted, RECONNECT can offer support such as:

  • advocacy
  • support to prepare for release from prison
  • help with moving back into the community
  • linking with healthcare and other services
  • peer support 
  • support to attend appointments (transport is not provided)
  • building confidence and self esteem
  • promoting independence
  • referrals to other services where needed

Who can refer to RECONNECT?

Referrals can be made for remand and sentenced prisoners. Referrals can come from the prisoner themselves, family / carers, Prison and Probation partners (i.e. offender management), substance misuse services, prison healthcare, Liaison and Diversion services. 

Please contact us to discuss the referral. Please also try to refer sooner, rather than waiting until nearer to the release date, to assist us to plan and create a robust support package.
