Community Nurses
We are a team of Specialist Learning Disability Nurses and support workers. We work across the city and county in four areas: City, South, North and West.
Our service is for adults aged 18 and over who have a confirmed learning disability (also called an intellectual disability). We work closely with other health and social care services.
How we can help
We can give you advice and support with your physical and mental health. We can assess your health needs and help you understand them. We may work with you on your own or in a group. We may also go to meetings to help plan the right support for you.
Community nurses can help with:
- Emotional and behavioural needs, such as positive behaviour support plans.
- Understanding your physical health, such as sleep, medicines and staying well.
- Getting the right support from health services.
- Mindfulness.

- Mental health needs, such as understanding a diagnosis, grief or bereavement.
We will talk with you about what support you need and agree how we can help.
This support may include:
- Working with your family and support staff
- Giving advice and information
- Working with social care and other services
- Working with other health professionals, such as your GP, hospital staff or dentist

Help in a crisis
Outside of office hours please contact your GP service, NHS 111, or in an emergency attend your local Emergency Department.
