We join up care, working together locally to provide care and security to people in our communities.

 


 

We will commit to

Integrate our mental and physical health services alongside partner services into neighbourhood working. 

 

We will deliver this by

Working with partners to redesign community physical healthcare and mental healthcare services around Integrated Neighbourhood Teams (INTs), including identifying the physical estate to deliver our services.*

Co-producing a 24/7 mental health neighbourhood care centre model to reduce the need for crisis escalation and hospital attendance.*

Working with partners, including the voluntary sector, to adopt a population health inequalities approach to inform neighbourhood service redesign for the benefit of our local population and communities.

Building on our services for Children, Young People and Families, connecting with other services to provide joined-up support.

 

We will know we are successful if

More people are using community services, which means fewer people need to be admitted to hospital for physical or mental health care.

The health inequalities gap by deprivation and ethnicity reduces.

 


 

We will commit to

Improve how we work together internally.

 

We will deliver this by

Creating flexible structures and ways of working across Care Groups and Corporate services, embracing matrix working to enhance the agility and connectivity of our services. 

 

We will know we are successful if

A way of working is established where organisations work together across local areas, and everyone knows who is responsible for patient care and clinical decisions.

 


 

We will commit to

Make partnership working everyone's job, ensuring services and information sharing is centred around the patient, not the organisation. 

 

We will deliver this by

Equipping and supporting our teams to work as equal partners with patients and other organisations to deliver joined up care. 

 

We will know we are successful if

Feedback from partners improves. 

Feedback from patients about their experience of joined up care improves.
